Diabetes Prevention Program

You are eligible to participate if you are at risk for Type 2 Diabetes. Risk factors include:

• Overweight
• Inactivity
• Close family member who has/had diabetes
• Older than 45
• History of gestational diabetes or had a baby weighing over 9 lbs.
• Fasting glucose over 100 or A1C >5.7%
• Diagnosis of pre-diabetes

The cost of the year-long program is $100.  Financial assistance available for partial or complete tuition costs.
